What to bring to your appointment

• Your insurance card(s)
• Your current eyeglasses, including your prescription sunglasses, occupational glasses, and any other prescription eyewear
• Contact lens information – bring your boxes or lens packages that lists the brand name, power, and base curve
• A list of your medications
• Completed paperwork, unless done online

We cannot bill both vision and medical insurance for exams on the same day. If you are coming in for a medical eye exam through medical insurance, you may be able to add on a refraction to determine your glasses prescription for an additional fee or return on another day for a vision exam through your vision insurance.
